Modern wellness is often reduced to diet charts, gym routines, and sleep trackers. But true well-being is much deeper. It is a balance of the mind, the heart, the body, and the way we live our daily lives.
To help individuals understand where they stand today—and what small shifts can elevate their well-being—we created the My Wellness Readiness Assessment, a simple 20-question tool that measures four essential pillars:
Mindfulness, Emotional Resilience, Physical Well-being, and Lifestyle Balance.
This assessment is quick, insightful, and designed to help people identify both their strengths and their blind spots.
A 20-Question Assessment to Understand Your Mind, Energy & Lifestyle**

1. Mindful Habits & Self-Awareness (5 Questions)
Focus: Understanding how aware and present you are in your daily life.
Mindfulness is not about long meditation sessions. It is about how often you come back to yourself—your breath, your emotions, your surroundings. Small habits like mindful eating, reflective journaling, or device-free moments can transform mental clarity and inner peace.
Sample Question:
“How often do you eat meals without distractions (TV, laptop, phone)?”
Options may include:
• Almost never
• Occasionally
• Often, I try to be mindful
• Most of my meals are device-free and present
Your responses reveal how connected you are to your inner world and how often you slow down enough to listen to yourself.
2. Emotional Resilience & Coping (5 Questions)
Focus: How you navigate stress, criticism, emotional triggers, and everyday setbacks.
Emotional resilience doesn’t mean avoiding difficult situations. It means responding with awareness instead of reacting with overwhelm. This section explores your ability to regulate emotions, handle criticism constructively, and apply self-compassion.
Sample Question:
“How do you respond when someone gives you unexpected criticism?”
Options may include:
• I get defensive immediately
• I feel hurt but try to hide it
• I listen but struggle internally
• I stay calm and separate feedback from self-worth
This dimension highlights your emotional toolkit and your capacity to bounce back with wisdom instead of stress.
3. Physical Well-being & Energy (5 Questions)
Focus: Your relationship with your body—not just workouts, but sleep, nutrition, and energy signals.
Well-being is not about strict fitness goals. It is about how energized, rested, and connected you feel to your body every day. This section explores your patterns of movement, rest, nourishment, and vitality.
Sample Question:
“How would you describe your relationship with physical activity?”
Options may include:
• It feels like a chore
• I do it only when necessary
• I enjoy it occasionally
• It is a regular, enjoyable part of my routine
The goal is to understand how supportive your physical habits are in creating long-term wellness.
4. Lifestyle & Balance (5 Questions)
Focus: How well you protect time for rest, relationships, hobbies, and joy.
In a world that celebrates busyness, balance is an act of courage. This section evaluates your boundaries with work, your ability to switch off, and your commitment to activities that nourish your spirit.
Sample Question:
“How well are you able to ‘switch off’ from work at the end of the day?”
Options may include:
• I can’t switch off
• I struggle to disconnect
• I can switch off with effort
• I have clear, healthy boundaries
Your answers help reveal whether your lifestyle supports harmony—or if constant pressure is draining your emotional and physical energy.
